WHAT YOU WILL NEED TO BRING:

Adult SIMPLIFIED RENEWAL (If you have an expired Canadian Passport issued underyour current name, in the last six years that has not been damaged, has neverbeen reported lost or stolen, was issued when you were at least 16 years ofage, and was valid for 5 years).

   1. Two identical passport photographs.  You are notrequired to have the photo signed by your guarantor, however, the name andaddress of the photographer or studio as well as the date the photo was takenmust be provided on the back of one photo. A photographer will be onsite totake your photos for a fee of $15. Please do not wear earings or white clothingas these photos will be refused by Passport Canada.

   3. Payment: For a 24 page passport $87 for adults For a 48 pagepassport $92 for adults. Passport Canada will accept a credit card (VISA,MasterCard, American Express), money order (postal or bank) or certified chequemade out to the Receiver General of Canada. NO CASH OR DEBIT CARD.

Note:

    * If you are applying more than 12 months prior to your passportexpiring please provide a written explanation as to why.
    * If you would like the submitted passport returned to you, youmust indicate this in section 2 of the form.
    * Your form and photos must be taken, completed & signedwithin the past 12 months.
    * Please sign within the borders of the signature box. Do nottouch the outer limits.
    * Your references (in Section 5) cannot be a family member andmust have known you at least 2 years.


Adult General Passport Application

   1. Proof of Citizenship: an original Canadian birth certificate orproof of Canadian citizenship.
   2. Secondary ID (e.g. health card, driver’s license, governmentissued identification. You will submit photocopies (both sides), and yourguarantor must sign & date the copies to indicate that the originals havebeen seen.
   3. Photographs: Two identical passportphotographs.  Your guarantor must write `I certify this to be thetrue likeness of (your name) `on the back of one of your photos, and sign. Thename and address of the photographer or studio as well as the date the photowas taken must be provided on the back of one photo.  A photographerwill be onsite to take your photos for a fee of $15. Please do not wear earingsor white clothing as these photos will be refused by Passport Canada.

Child General Passport Application

   1. Proof of Citizenship: an original Canadian birth certificate orproof of Canadian citizenship. To avoid possible delays, we strongly recommendyou provide a long-form birth certificate showing information on both parents(this is required if the child was born outside wed lock).
   2. All documents that refer to custody or mobility of, or accessto, the child. Both parents are requested to participate in obtaining passportservices for their child and to sign the application form. Where a court orderor agreement exists referring to custody of the child, only the person withcustodial rights may apply. If a divorce has been granted, a copy of thedivorce judgement or order must also be provided.
   3. Photographs: Two identical passportphotographs.  Your guarantor must write `I certify this to be thetrue likeness of (your name) `on the back of one of your photos, and sign.    5. Payment: For a 24 page passport $37 for children (ages 3-15),$22 for children (under the age of 3). For a 48 page passport $39 for children(ages 3-15), $24 for children (under the age of 3). Passport Canada will accepta credit card (VISA, MasterCard, American Express), money order (postal orbank) or certified cheque made out to the Receiver General of Canada. NO CASHOR DEBIT CARD.

Note:

    * Your form completed & signed within the past 12 months.
    * For children under 12 months: To ensure the photo is a currentand true likeness of the child, it must be taken within one month of the datethe application is submitted.
    * Children 11 years or older must sign. Please sign within theborders of the signature box. Do not touch the outer limits.
    * The guarantor`s signature of your application (section 2) andone of your photos must be dated the same day or after the dates of yoursignatures.
    * For children under 3 years their passport will be valid for 3years. For children ages 3 to 15 their passport will be valid for 5 years.Children who have been issued a passport within their first 12 months may beissued a one-time replacement passport, free of charge, valid for the balanceof three-year validity period of the original passport.
