如果父母有一方或者双方没有随同小孩旅行,需要办理授权信,模板如下.最好能请律师或公证机关公证一下($20-$30). 将以下内容拷贝到WORD，调整一下格式即可。
Note: It is strongly recommended that children   travelling alone orwith one parent carry a consent letter for each and every   tripabroad.Although anyone can witness/sign these letters, it is advisable to havethe consent letter certified, stamped or sealed by an official who has theauthority to administer an oath or solemn declaration (i.e., a commissioner foroaths, notary public, lawyer, etc.) so that the validity of the letter will notbe questioned. It is up to each official/individual who witnesses such a letterto decide what proof he/she needs to see to be able to witness/sign the letter.Official sshould only witness/sign a letter of consent if he/she is convincedthat the individual requesting the letter is who he/she claims to be and thatadequate proof has been provided.
                  
              This consentletter may be required by foreign authorities among other additionalcountry-specific entry requirements. You should contact the representatives ofthe country or countries to be visited by the child to ensure that you have themost up-to-date information regarding their specific entry requirements aspermission to enter another country is the sole prerogative of that country.
                We alsostrongly recommend that you contact the transportation company (airline, train,bus, etc.) in order to observe any additional policies that they might have inplace.
